Liability Waiver *

I, the undersigned client, hereby acknowledge that I have voluntarily chosen to board my dog with Ryan Bullard, herein referred to as "Ryan's K9 Companion" and I agree to the following terms and conditions: Health and Vaccinations; I certify that my dog is in good health and has received all necessary vaccinations, including but not limited to rabies, DHPP (Distemper, Hepatitis, Parvovirus, Parainfluenza), and Bordetella (kennel cough). I understand that proof of vaccination may be required, and I agree to provide such proof upon request. Behavior; I understand that my dog will be interacting with other dogs and humans during their stay at Ryan's K9 Companion facility. I certify that my dog does not have a history of aggressive behavior towards other animals or humans. I agree that if my dog displays aggressive behavior, Ryan's K9 Companion reserves the right to isolate my dog from others and to take any necessary actions to ensure the safety of all animals and humans on the premises. Emergency Veterinary Care; In the event of a medical emergency, I authorize Ryan's K9 Companion to seek veterinary care for my dog. I agree to be responsible for all veterinary expenses incurred, including but not limited to examination fees, treatment, medication, and transportation costs. Payment; I understand and acknowledge for dog boarding services payment must be made in full prior to the commencement of the boarding period. The payment is due upon booking confirmation and must be received before the pet is dropped off for boarding. Failure to provide payment in advance will result in the cancellation of the boarding reservation. Cancellation Policy; Clients must provide at least 7 days' notice prior to the scheduled boarding start date for a full refund. Cancellations made with less than 7 days' notice will not be eligible for a refund. This policy ensures we can efficiently manage our bookings and accommodate other clients. Assumption of Risk; I understand and acknowledge that there are inherent risks associated with dog boarding, including but not limited to injury, illness, or escape. I assume all risks associated with boarding my dog at Ryan's K9 Companion's facility and hereby release Ryan's K9 Companion, its employees, agents, and representatives from any and all liability for any injury, loss, or damage to my dog or personal property. Indemnification; I agree to indemnify and hold harmless Ryan's K9 Companion, its employees, agents, and representatives from any claims, damages, or liabilities arising out of or related to my dog's stay at Ryan's K9 Companion's facility, including but not limited to any claims brought by third parties for injuries or damages caused by my dog. Photography and Videography; I grant Ryan's K9 Companion permission to photograph or video my dog during their stay for promotional or documentation purposes. I understand that these images or videos may be used on Ryan's K9 Companion's website, social media platforms, or other promotional materials. Agreement Acknowledgment; I have read and understand the terms and conditions outlined in this Dog Boarding Liability Waiver. I agree to abide by these terms and acknowledge that failure to do so may result in the termination of my dog's stay at Ryan's K9 Companion's facility.
